Thank you for choosing Dr. Laura T. Muller and The Eye Center for your eye care needs!
On your first visit with Dr. Muller, you will be asked to complete an array of forms. Completing these forms beforehand will expedite your first visit to our office.
If you have access to a printer, we are providing these forms for you so you can fill them out in advance and bring them with you to your first visit.
What to bring on your first visit:
- If able, the following completed forms:
- Your current medical insurance card(s).
- Knowledge of your eye and medical history.
- A list of all of your current medications (both eye medications and any other prescriptions you currently take.)
- The date of your last eye exam and past eye exam records (if available).
- Your current glasses or contacts.
- A pair of sunglasses for your trip home. Your eyes will be dilated and light sensitive for a few hours after the visit. Some patients feel more comfortable having a driver with them to take them home after the visit.
Please complete the Medical Records Release form if our office needs to obtain records from another eye care provider. |

Dr. Laura Muller is a participating provider with most major medical health insurance plans. We have provided a list of our current medical insurance plans. The Eye Center is always adding new insurances. If you don't see your plan listed, please call our office at 727-216-2020 to see if we are participating with your medical insurance.
Medical Insurance Plans
It is the policy of The Eye Center to collect co-payments and deductibles at the time of service per the specifics of your medical insurance plan. Payments may be made by cash, check, MasterCard or Visa. For procedures not covered by your medical insurance, our Billing Specialist will work with you to create a payment plan.
As a courtesy, The Eye Center will file your primary and secondary insurances. We ask that you always keep our office updated with any name, address or medical insurance changes so we can assist you with your claims.
